Are there any parts of the FHIR spec that allow listening to change events from the FHIR server (with potential filtering)? So you could build a client that watches and syncs with a server?

There is http://hl7-fhir.github.io/subscription -- which takes a stab at defining websocket notifications. But this hasn't been implemented outside of reference servers, AFAIK.
